Subject: Employment Ontario Forms
OBJECTIVE:
The purpose of this bulletin is to communicate to service providers delivering Employment Ontario (EO) programs that both new and revised forms have been posted to the EOPG.
Service providers delivering the ES, LBS, YJC, YJCS or YJL programs should become familiar with the use of these forms.
For LBS e-Channel service providers, MTCU will work with you over the following weeks to incorporate these changes into your online systems.
BACKGROUND:
The Employment Ontario (EO) Participant Registration Form (3006) will be used for registering participants in the following programs:
The Employment Ontario (EO) Employer Registration Form (3004) will be used for registering employers in the following programs:
The user must select a program option located at the top of the form The relevant program content will be displayed in addition to the common section.
The form is fillable, saveble and printable. In order to view/print the forms, service providers will need Adobe Acrobat Reader (version 9.0 or above) installed on their computers.
